The cause of death for 66-year-old Eugene Gates Jr. was listed as hypertensive and atherosclerotic cardiovascular disease and hyperthermia.

The Occupational Safety and Health Administration is investigating Gates’ death. Lewis said the union conducted its own investigation and found that his heat training records were marked as completed on a day in January he was not scheduled to work.
